Chapter 04 / Your questions
Ask until the responsibility has a name.
You don't need to sound like an inspector. These questions are for a conversation about your care. Use your own words and leave room to write down the answers.

Get the name and license.
“Who will give the injection, and what kind of license do they hold?” Check MDs with the Medical Examiners board, DOs with the Osteopathic Medicine board, and nurses with the nursing board. Physician assistants may be licensed by either medical board.
Ask about the people involved in your care. “A nurse” gives you a role, but you'll need a full name to check a license. Write down both rather than trying to remember later.
The Nevada rules page covers who may give these injections and where. The supervision page explains the nursing roles and supervision terms. Keep either page handy if it helps you frame a follow-up.
Ask two questions about the product.
Where did it come from?
“What exact product will you use, and how do you check its source?” For wrinkle-relaxing botulinum toxin injections, FDA says to confirm an authorized source. Ask for an answer about the product proposed for you.
How is it handled?
“Are needles and syringes used only once? How do you prevent contamination when taking medication from a container?” CDC's injection safety advice explains these handling checks.
Keep the two answers separate in your notes. A product name doesn't explain the handling. If an answer is confusing, ask for the everyday version rather than nodding along.

Say the question once, away from the room.
You can talk through a question with someone you trust, or bring it on paper. If it concerns fillers, say which part of the face you mean. That helps keep the supervision question specific. There is no special wording to get right.
“What are the risks and alternatives for me? Who can I reach afterward? Which symptoms need urgent care?” Ask the clinician assessing you for personal answers, and write down the contact details.
You can leave a question open.
Take time to think if something is missing. Ask the relevant board about unclear license information. Bring your notes when asking a follow-up, so you can point to the exact gap. For a concern about professional conduct, the official complaint routes are collected here.
