Contact & concerns

# The concern belongs with someone who can act on it.

Pure Med Spa LV gives no medical advice and sells nothing. For a license question or concern about professional conduct, use the official board routes below. This guide doesn't receive clinical messages. There is no form or upload channel here.

## Get help for a health concern.

For a personal question about a procedure, contact a licensed U.S. clinician who can assess you. Ask about the risks and alternatives that matter in your case. Bring any unanswered questions from your earlier conversation.

### Choose the right conversation

A clinician can address your health. A licensing board handles matters within its authority. Use the route that fits the concern you have.

After a wrinkle-relaxing botulinum toxin injection, symptoms such as double vision, trouble swallowing, shortness of breath or weakness need medical attention. FDA's [safety alert](https://www.fda.gov/drugs/drug-alerts-and-statements/counterfeit-version-botox-found-multiple-states) directs people with these symptoms to a health professional or emergency room. Don't wait for a complaint response.

## Match the board to the license.

### Medical Examiners

The Nevada State Board of Medical Examiners' [website](https://medboard.nv.gov/) has license lookup and disciplinary information for people it regulates. Its [complaint page](https://medboard.nv.gov/Patients/File_a_Complaint/) offers online and PDF routes. Read the instructions on that page.

### Osteopathic Medicine

For an osteopathic doctor (DO), use the [Nevada State Board of Osteopathic Medicine](https://www.bom.nv.gov/). Its website links Verify a License and File a Complaint. Physician assistants may be licensed by either medical board, so check which board holds their license.

### Nursing

The Nevada State Board of Nursing links a [license lookup](https://nevadanursingboard.org/) and a page for [nursing complaints](https://nevadanursingboard.org/submit-a-complaint/). Use the lookup to check an active license or certificate, and the complaint page for its submission instructions.

If you aren't sure which board applies, get the person's full name and license type first. Don't treat a missing result in one lookup as the end of the search.

## Write down what happened.

The nursing board requests the nurse's details, incident dates and location, a specific account and lawfully obtained supporting documents. It investigates before deciding. Filing a complaint doesn't establish that someone did something wrong.

### Stick to what you know

Separate what you saw, what someone told you and what remains unclear. Keep the questions beside your notes rather than guessing at an answer.

Read the board's instructions before sending personal or medical information. Use its own submission route, and check what documents it requests. Keep the professional's name and license type beside your account, so you can explain who was involved. If a detail is uncertain, say so.

If you need an explanation clarified, the [question sheet](/questions) can help you find the missing part. The [references page](/references) keeps the source wording within reach.

Pure Med Spa LV is a reading guide; it runs no clinic or pharmacy and sells nothing.
